Provider Application Form

At Ease USA (AEU) is looking for licensed trauma providers and/or organizations who practice evidence-based therapy. We are looking for providers who have experience with the military and veteran communities, and their family members. If you are interested, please complete the following application to become part of the Preferred Provider Network (PPN).

At Ease USA Commitment & Practices 

  • If the client does not have active insurance, or if the provider is not in network with the client’s insurance plan, AEU will be the payor of last resort. 

  • AEU will reimburse to the Provider within 30 days of Provider’s invoice submission and monthly update report(s).

  • Refer prospective clients to mental health providers in the AEU network, matching client to therapist based on geographical location and other client requests. 

  • Assist any prospective client requesting mental health services to obtain a timely referral to a mental health provider with a linkage agreement to AEU. 

  • Actively promote all AEU providers through the AEU website and social media channels.  

  • AEU will use best practices when storing confidential client information.

  • AEU will provide an IRS form 1099 to provider no later than January 31 of each year.

  • AEU clinical director will provide consultation and support for providers if and when needed. 

  • AEU will collect data from the clients to help identify treatment progress. This data will be shared with the provider when the data and assessments are completed by the client. 

  • AEU will provide access to forms and updates on the AEU website. 

Termination

  • Either party may terminate this Agreement by providing the other party with written notice of termination at least 30 days in advance of the date of such termination.  

  • In the event of termination for any reason neither party shall have any further obligation to the other under this Agreement.
